Institutional Reporting

Colleges, universities and other institutions of postsecondary and higher education are required to submit reports annually to the Pennsylvania Department of Education (PDE) on several important topics. Below are links to the various laws and regulations, information about reporting requirements, and forms that can be downloaded.

The following reports are due to be submitted to the Bureau of Postsecondary and Adult Education on their respective due dates.

English Fluency in Higher Education Act

The Act: English Fluency in Higher Education Act (PDF)

Form: English Fluency Assurance of Compliance Form (PDE-5032) (PDF)
Due Date: September 1 annually

Higher Education Gift Disclosure Act

The Act: Higher Education Gift Disclosure Act (PDF)
Forms: Sworn Statement of Nationality Form 2016-2017 (PDF)
The Higher Education Gift Disclosure Report Form 2016-2017 (PDF)
Due Date: Not later than the due date for the federal return of organizations exempt from income tax
Note: Form is submitted only if gifts are reportable.

Article XX-G Sexual Violence Education at Institutions of Higher Education

Due Date: September 1 annually

Uniform Crime Reporting Act

The Act: Uniform Crime Reporting Act (PDF)

Form: Assurance of Compliance Form (PDE-3153) (PDF)
Due Date: Please file this form to the office below no later than March 1 annually.

Graduation Rates for Colleges (Act 88 of 2002)

Note: Currently does not apply to for-profit institutions, 2 year institutions or seminaries.

Annual Survey of Educational Programs (ASEP)
